Central Coast Home Health is a fast-growing and locally owned organization with an excellent reputation throughout San Luis Obispo and Santa Barbara counties. We are constantly growing and now seeking a Part time or Full time Palliative Care Case Manager to join our team in the Nipomo, Santa Maria, and Lompoc areas.

A Palliative Care Nurse cares for patients with life-limiting diseases who are still seeking curative treatments and interventions. The focus is on symptom management, advance directives and other end of life decisions, and education regarding disease processes. The Palliative Care nurse works collaboratively with a multidisciplinary team including social workers, spiritual counselors and physical/speech/occupational therapists. By seeing your patients in the home, you are able to spend the time necessary developing relationships, answering questions and assessing the needs of the patient and family on a psychosocial, physical and spiritual level.

Regular work schedule is Monday through Friday, 8am-5pm with a choice between 4 or 5 days/week and a weekend rotation approximately every 4 weeks.
