Responsibilities

We are seeking a Full-time LPN to work two 16-hour shifts weekly, from 7am to 11:30pm, and includes every other weekend and holidays. 

Rockford Center is a 138-bed mental health facility located in Newark, DE, providing easy access to a full complement of inpatient and partial behavioral health programs for children and adolescents, adults, and older adults. Rockford is the only child inpatient behavioral health facility in DE, offers an experienced and tenured senior management team, and serves New Castle County and northern Maryland.

Position Summary:

The LPN/LVN at Rockford Center administers prescribed medications, maintains documentation for quality improvement and infection control, educates patients and families on medications, completes nursing assessments under RN guidance, and transcribes medication orders accurately and promptly.

Responsibilities:

  • Helps identify patient problems, needs and strengths, and suggesting modifications to the treatment plan
  • Completes Adverse Drug Reaction forms and forward to the Pharmacist
  • Maintains patient, program, department and related documentation
  • Collects and analyzes data regarding the patients physical, psychosocial, environmental, self-care, educational and anticipated needs throughout treatment
  • Conducts and documents ongoing patient evaluation tools
  • Medication administration and management of patient medications and physician orders
  • Participates as a member of interdisciplinary treatment team
  • Provides appropriate nursing intervention/documentation in crisis situations
  • Reports unsafe patient conditions and change in status
  • Provides age-specific communication, limit setting and interventions
  • Prepares and adheres to the facility patient discharge process
  • Assists patients in carrying out activities of daily living
  • Participates in hospital emergency codes necessary to the unit’s and the hospital’s safe operation

Qualifications

Requirements :

  • Attendance of accredited vocational school for LPNs and a current State of Delaware LPN License.
  • Current CPR or BLS certification.
