Job Description
We are looking for a passionate and compassionate Bilingual (Spanish-speaking) Care Manager to join our team! This position offers a hybrid work schedule, allowing you to work from home with occasional in-person requirements. You'll be part of a supportive and dynamic team dedicated to helping individuals with intellectual and developmental disabilities (I/DD) live their best lives.
Key Responsibilities:
Conduct comprehensive assessments to determine the needs, preferences, and goals of individuals.
Collaborate with an interdisciplinary team to create a personalized plan of care, addressing both clinical and non-clinical needs.
Facilitate access to services, including health management, chronic disease care, and long-term care.
Work closely with individuals and their families to ensure services are culturally and linguistically appropriate.
Advocate for individuals, empowering them to make informed choices and support their independence.
Utilize technology to maintain accurate documentation and communicate effectively within the care coordination team.
Ensure compliance with HIPAA and all relevant state and federal regulations.
Participate in ongoing training and development to enhance your skills and knowledge.
Ideal Candidate:
Education: A Bachelor's degree in a related field OR a valid RN license OR a Master's degree with relevant experience.
Experience: At least two years of experience in a care management, case management, or related field. Experience working with individuals with I/DD is preferred.
Skills: Strong time management skills, the ability to work independently, and excellent communication abilities. Familiarity with health records and technology is important.
Languages: Fluency in Spanish is required to ensure effective communication with clients and their families.
